2 incidents come to mind. 

In bmt, a guy is so unstable that during run down,  2 officers ran with him on each side just to make sure nothing... NOTHING!!!... happens. 

In school,  during live firing exercise where we form up line and charge last burst.  One fella managed to fire in such a way his bullets were kicking up dirt right beside his neighbor.
