  1. Volkswagen and Audi Recall 261,000 Cars Over Fuel Pump-Related Fire Risk - TFLcar Looks like America has a recall on Golf 7. GTi is affected. Possible fire hazard. This fuel pump recall for certain VW Group vehicles mirrors an earlier 2016 campaign to address the issue. Volkswagen and Audi have launched a new recall campaign across most of its Mk7 Golf-based vehicles for a potential fire risk due to faulty seals in the suction jet fuel pump. Located inside the fuel tank, some pumps were manufactured with too much tension on the suction jet pump seals. Their pre-damaged condition could allow fuel to leak out into the evaporative emission control system (or EVAP), where it could damage the charcoal canister element or, in extreme cases, find an unwanted ignition source and start a fire. The recall covers these cars based around the past-generation Golf’s platform: 2015-2020 Audi A3 Sedan (47,391 units) 2015-2019 Audi A3 Convertible (Cabriolet) (3,875 units) 2015-2020 Volkswagen Golf (58,544 units) 2015-2020 Volkswagen Golf GTI (114,485 units) 2015-2017 Volkswagen Golf SportWagen (19,673 units) 2018 Volkswagen Golf SportWagen GP (606 units) 2018-2019 Volkswagen Golf SportWagen A7 (facelifted version; 5,758 units) 2019-2020 Volkswagen Jetta GLI (10,925 units) All affected vehicles are front-wheel drive, and have the “Version 1.0 or 1.1” suction jet pump installed. This recall campaign does not extend to all-wheel drive models like the Volkswagen Golf Alltrack, Golf R or the Audi A3 Quattro, according to what the automaker told the National Highway Traffic Safety Administration (NHTSA). Telltale signs that the fuel pump seals have failed include problems refueling where the pump shuts off before the tank is full, or if drivers notice gasoline smells when there’s no evidence of a spill after filling up. An earlier attempt to address this issue in a 2016 recall involved using new pumps with the same design, so the campaign did not ultimately solve the issue.

HSA recalls 3 brands of high blood pressure drugs over potentially harmful impurity SINGAPORE: Three brands of high blood pressure medicine containing the ingredient losartan have been recalled because they contain higher than acceptable levels of a potentially harmful impurity, the Health Sciences Authority (HSA) said on Thursday (Mar 28). The drugs contain the active ingredient losartan potassium, which was manufactured by Indian pharmaceutical company Hetero Labs. The affected products are the 50mg and 100mg tablets from the brands Losartas, Losagen and Hyperten and distributed by local suppliers Apotheca Marketing, Medicell Pharmaceutical and Goldplus Universal, respectively. About 137,000 patients in Singapore are using the three recalled brands of losartan medicine, said the Ministry of Health (MOH). Losartas is prescribed in public healthcare institutions, while Losartas, Losagen and Hyperten are prescribed at private healthcare institutions. HSA has advised patients not to stop treatment on their own as there is no immediate health risk and sudden stopping of the drugs can pose greater immediate risk to their health. The recalled products were found to contain trace amounts of a nitrosamine impurity, N-nitro-N-methyl-4-aminobutyric acid (NMBA), which are above internationally accepted levels, HSA said. Exposure to nitrosamines at high quantities over a long-term period may potentially increase the risk of cancer. For example, the added cancer risk from an additional six-month exposure is estimated to be less than 0.0002 per cent. “The risks of trace amounts of NMBA are associated with long term exposure. Sudden stopping of the medicines can pose greater and more immediate risk to patient’s health … We have advised healthcare professionals to review the medicine and treatment plans of their patients,” HSA said. HSA said that several losartan medicines have been recalled overseas since end-February due to the presence of NMBA. It tested all locally marketed losartan products for the presence of the NMBA impurity and in March found the three brands contained trace amounts of NMBA above acceptable levels. The other seven brands of losartan medicines marketed in Singapore are NOT affected by this impurity.

  4. This thread served to provide a central reference for vehicle recall issued by automotive manufacturer. For a start, I will start with something light (but still along the line hor). Case #1: Lamborghini Centenario It doesn’t matter how rare or expensive your car is, there is always a chance it could be recalled. The Lamborghini Centenario was developed to pay homage to the 100th birthday of Lamborghini’s founder. It’s based on the Aventador and is powered by a naturally aspirated V12 engine, generating 770 horsepower. Only 40 units total are out in the wild – 20 coupes and 20 roadsters – and apparently at least 11 of them are in the U.S. The 2017 Lamborghini Centenario is being recalled due to an incorrect certification label, which may have an incorrect weight limit. This could cause the vehicles to potentially be overloaded, increasing the risk of suspension or tire failure, which could increase the risk of a crash. A total of 11 units are affected by the recall, which includes both the Centenario coupe and roadster models. The Italian automaker will begin notifying owners on how to get the correct label fitted to their exotic. Err... how much could one possibly overload a 2 seater sport car? Come on, no 1 owner will likely be using it to say transport heavy items, of for the matter of fact, use it as a cargo mover, so how can it get overloaded to the extend of having it suspension damaged?? Well, I guess that is a high SES question that low SES people like me cannot foresee. Case #2: Rolls-Royce Ghost Amid a seemingly endless run of multimillion-vehicle recalls by volume carmakers, Rolls-Royce looks to have joined an elite group for making one of the smallest. The British-based luxury carmaker was forced to issue a safety recall notice in the US because of a problem with the side-impact airbags on the 2015 model of its best-selling Rolls-Royce Ghost. The number of cars affected — one. The company, which is owned by Germany’s BMW, told the Financial Times that the issue “was due to the incorrect labelling on one of the airbags” that had been identified during the customer pre-delivery check by Rolls-Royce. The company added that at no point was there any question of a safety risk. “It is a good way of demonstrating Rolls-Royce’s attention to detail,” it said. The carmaker, which sold a little more than 4,000 vehicles last year, said the car with the fault had left its factory in Goodwood, West Sussex, in January 2014 but its North American owner had not yet taken delivery. “The affected vehicle has thorax air bags fitted to both front seats that may fail to meet the side impact performance requirements for the front seat occupants,” according to the recall notice, issued by BMW North America, through the US Department of Transportation’s National Highway Traffic Safety Administration. Rolls-Royce did not confirm which company supplied the airbags. Although the NHTSA did not immediately respond to a request for comment, subsequent data provided by the agency confirmed that among carmakers a single vehicle recall is rare. It is label again... Now, since the recall involved only 1 car, and that car was not even delivered to the customer, is there a need to issue a statement of recall for it?   5. Japan's Honda Motor on Monday recalled more than two million vehicles worldwide over an airbag defect that could pose a fire risk. Honda said the recall covers about 2,033,000 vehicles which were produced between August 2000 and December 2005, including over one million in North America and 668,000 in Japan. Subject to the recall are a total of 13 types of vehicles including popular Fit and Accord models. Front passenger airbag inflators could have been assembled with an improperly manufactured propellant component, Japan's third largest automaker said. That could cause the container of the inflator to rupture in the event of a crash, posing a fire risk or injuring passengers, it added. The same inflator problem has also caused top automaker Toyota to recall millions of vehicles globally. Toyota has earlier said it acted as it received a complaint from a Japanese customer who said his passenger seat was burned from the defect.   6. Hope all are clear! National University Hospital recalls 178 children for TB tests The first of the 178 patients were at NUH (above) for screening on Tuesday. The children will have a chest X-ray to check for TB, and blood tests if aged five or older, and/or skin tests to see if they have the bug latent in them. The National University Hospital (NUH) is recalling 178 paediatric patients - including 131 under the age of two years - who had been cared for by a nurse now confirmed to have tuberculosis. They include 34 children who have received a transplanted organ. These children are on immunosuppression drugs and therefore at higher risk than normal children. Associate Professor Daniel Goh, head of paediatrics at NUH, said the risk of any of the children contracting TB from the nurse is "very low", especially as she had donned a mask while working. But the hospital is not taking any chances and is recalling patients who had been in ward 47 since July. The nurse was treated at a GP clinic for her cough in July, but despite having a chest X-ray, was not diagnosed with TB. She was given antibiotics and seemed to get better. NO CAUSE FOR ANGER I would be angry if the hospital did not do anything and the patients started getting TB. But they are doing something. MADAM CRYSTAL LIM, 28, whose son was in ward 47 for three weeks in August. Her son had a liver transplant in October last year, but was back in hospital for treatment for a 3cm abscess on his buttocks. The doctor had explained that because of his transplant, he was at a higher risk. Madam Lim is worried, but not angry. How the disease is spread Tuberculosis is an airborne disease transmitted through fine respiratory droplets from an infected person. It usually affects the lungs but can also hit other parts of the body such as the brain, lymph nodes, kidneys, bones and joints. Persons with TB are most likely to spread it to people who have close and prolonged contact with them, such as family members, friends and colleagues. It is not spread through sharing drinks, utensils, cigarettes or kissing. There were 1,454 new TB cases among residents here last year, 34 more than in 2013. In the past 30 years, Singapore had more than 1,000 new cases a year. According to the Health Ministry, there has been a resurgence of TB cases in recent years. TB can be cured with a combination of different drugs taken over six to nine months. However, if the treatment is incomplete, TB germs may develop a resistance to the drugs. Second-line TB drugs must then be taken for a longer period, but chances of a cure will be significantly reduced. Tan Weizhen But the cough persisted, and last Wednesday she sought treatment again and was given a CT scan which showed a possible TB patch in her lung the size of a 50-cent coin. She told the hospital on the same day and was tested for TB. Last Friday, the results confirmed she had TB. Fortunately, it is the normal and not multi-drug-resistant variety. Dr Goh said his team spent the weekend trawling through the patient database to identify patients who might have had long exposure to her while warded, as well as those who are deemed at higher risk because of their age or disease. Mr Joe Sim, NUH's chief executive officer, said: "We fully understand the anxiety of the parents and are taking this matter seriously." Professor Paul Tambyah , a senior infectious disease expert at NUH, said TB transmission depends on the amount of exposure, the bacterial load of the carrier and the individual's immune system. He said there is a one in 10 chance of people getting the bug if a person with TB coughs at them for two hours. Of those who get the bacteria, one in 10 would get the disease in his lifetime. This doubles for those with low immunity. The first patients were at NUH for screening on Tuesday and more will be screened over the coming weeks. They will have a chest X-ray to check for TB, and blood tests if aged five or older, and/or skin tests to see if they have the bug latent in them. Any child diagnosed to have caught the latent bug will be given treatment, which has a greater than 90 per cent chance of preventing TB. For those with TB, chances of a cure are very high with treatment. Madam Crystal Lim, 28, whose two-year-old son was in ward 47 for three weeks in August, was shocked when his doctor called to ask her to bring her son back for tests. Her son had a liver transplant in October last year, but was back in hospital for treatment for a 3cm abscess on his buttocks. The doctor had explained that because of his transplant, he was at a higher risk. She is worried, but not angry. "I would be angry if the hospital did not do anything and the patients started getting TB. But they are doing something," she said. As for the nurse, Madam Lim said: "She did not do it purposely. All the nurses look after the children very well. I hope the doctors can help her get well." The nurse, who is on medical leave for two weeks, will be able to resume work as being on treatment means she is not contagious. Her colleagues in the ward have all been tested and found negative. Local distributor Nature's Farm recalls health supplement for men SINGAPORE - The Health Sciences Authority (HSA) on Monday issued a health advisory, alerting members of the public against consumption of male performance enhancement drug "VIA.X For Men". Nature’s Farm Pte Ltd, the local distributor of the product, has recalled the product from the market. HSA said in a press statement that it had found "undeclared potent ingredients in the product" and is advising all consumers who may have the product to stop consuming it immediately. "VIA.X For Men" claims to contain only natural ingredients, including Red Algae Peptide Hydrolysate, Pycnogenol, L-Arginine and Zinc Gluconate. However, samples of the product were tested and found to be adulterated, containing undeclared potent ingredients of sildenafil and propoxyphenyl sildenafil. "Sildenafil is used to treat erectile dysfunction and should only be used when prescribed by a doctor. Its usage without proper medical advice and close supervision can cause serious adverse reactions such as decreased or loss of vision and hearing, low blood pressure and other cardiovascular events such as stroke and myocardial infarction," HSA said in the statement. "In addition, sildenafil is not suitable for patients with some medical conditions including certain heart problems or patients who are taking heart medications such as nitrates." Propoxyphenyl sildenafil is a chemically related compound of sildenafil. It is not approved for use as a medicinal ingredient and could cause adverse effects which are similar to those of sildenafil. HSA said that they have not received any reports of adverse reactions associated with the product. Consumers who have taken the product should seek medical attention if they are feeling unwell or concerned about their health. Business Times - 24 Mar 2012 Volvo recalls more than 12,000 cars in China BEIJING - Volvo Cars is recalling 12,798 of its 2012 model year vehicles imported into China, the top Chinese quality regulator said, in order to prevent potential fuel leaks and to fix faulty wiring that could cause airbag failure. Volvo is recalling 11,119 S60 and XC60 cars to fix a wire harness under the front seats which might interfere with the car's airbags, China's General Administration of Quality Supervision, Inspection and Quarantine said on its website late on Friday. The firm is also recalling 1,679 S60 and XC60 cars to inspect the underbody coating which may penetrate the fuel lines and cause fuel leakage, it added. No casualties or deaths have been linked with the potential defects, the watchdog said. Geely, parent company of Geely Automobile Holdings Ltd , took over Ford Motor Co's Volvo car unit in August 2010. It was not immediately available for comment.
  10. BMW has issued a recall of five different model lines covering the years 2008-2011 due to a fire risk. Turbocharged eight- and twelve-cylinder engines in the 5 Series, 5 Series Gran Turismo, 7 Series, X5 and X6 could suffer from overheating circuit boards that control the electric auxiliary water pump. The overheating board could cause the pump to smoulder, and in extreme cases among the 102 failures, BMW has found there were two engine compartment fires.   13. We won't blame you for reading this headline and thinking, "Oh great, another Toyota recall." After all, the Japanese automaker dominated headlines in 2010 by recalling every single model in its then-current product portfolio, including the massive sticky pedal and unintended acceleration recalls that took place in the first half of last year. Now, Toyota has issued a voluntary recall on about 245,000 Lexus sedans here in the United States and an additional 1.5 million vehicles around the world. Here in the U.S., the 2006-2007 Lexus GS 300 and GS 350, 2006-2009 Lexus IS 250 and 2006-2008 Lexus IS 350 sedans are being recalled due to faulty installation of fuel pressure sensors. If this sensor has not been fastened tightly enough, fuel has the potential to leak between the gasket that connects it to the fuel delivery pipes. Including these North American vehicles, the fuel sensor recall affects 354,524 vehicles around the world, with Automotive News citing that the Toyota Crown and Mark X sedans are being recalled in the automaker's home market. Toyota's other global recall involves faulty fuel pipes and fuel check valves, though none of the 1.34 million vehicles affected by this one are in North America.   14. Honda Motor said yesterday it would recall about 1.35 million Fit sub-compacts globally to repair defective wiring in the headlights. No accident was reported from the defect, a Honda spokesman in Tokyo said. Cars in Singapore will be affected, according to a preliminary check. The exact figure will be released following a proper tally. Subject to the recall are Fit cars built at Honda's Suzuka factory in Japan between November 2001 and October 2007. About 735,000 units will be recalled in Japan, where the Fit is the second most popular car behind Toyota Motor Corp's Prius, excluding 660cc mini-vehicles. In Japan, the recall will cost about 3.6 billion ($56 million) and will have negligible impact on Honda's earnings, the spokesman said. Honda will also recall 143,000 Fits exported to the United States, and 385,000 units in Europe, where the model is called the Jazz. The car will also be recalled in the Middle East, Africa and Asia.   16. Toyota: 3.8 million cars with risky floor mats NHTSA and world's largest automaker warn vehicle owners to remove driver's side floor mats in 7 Lexus and Toyota models. NEW YORK (CNNMoney.com) -- Toyota issued a safety warning for 3.8 million Lexus and Toyota cars because of potentially deadly floor mats. In statements released Tuesday, the world's largest automaker and the National Highway Traffic Safety Administration warned drivers to remove the mats or risk a forced-down accelerator pedal that could lead to a fatal crash. The seven models being recalled could potentially have removable mats that would interfere with the pedal and cause it to stick. "A stuck open accelerator pedal may result in very high vehicle speeds and make it difficult to stop the vehicle, which could cause a crash, serious injury or death," said Toyota in its statement. Though Toyota recalled accessory all-weather floor mats in 2007 for similar problems, NHTSA noted that reports of vehicles accelerating rapidly even after the release of the pedal have continued. In a statement posted on the automaker's Web site earlier this month, Toyota said that preliminary information from investigators indicated that a fatal crash involving a family driving a 2009 ES350 loaned by a local Lexus dealer in San Diego could have been caused by an all-weather floor mat interfering with the accelerated pedal because of a faulty mat installation. "There is an urgent matter," said U.S. Transportation Secretary Ray LaHood in the NHTSA statement. "For everyone's sake, we strongly urge owners of these vehicles to remove mats or other obstacles that could lead to unintended acceleration." Toyota has announced that it will soon launch a safety recall to redress the problem, but NHTSA and the automaker warn Toyota and Lexus drivers of certain models to remove that floor mats sooner for safety. The Toyota models being impacted are: 2007-2010 Camry, 2005-2010 Avalon, 2004-2009 Prius, 2005-2010 Tacoma and 2007-2010 Tundra. The Lexus models facing the safety recall are the 2007-2010 ES 350, and the 2006-2010 IS 250 and IS 350.

Porsche and the National Highway Traffic Safety Administration are issuing a recall for the 2010 Panamera for a potential seat belt defect that could leave the driver unprotected in the event of a crash. NHTSA says the defect can occur when either front seat is put in an "extreme position," which could lead to the seat belt mount being detached from the anchoring mechanism while the belt is fastened or opened. The post-jump NHTSA release doesn't explain what an "extreme position" is, though we're guessing that the safety administration is referring to a seat that is either very close to or very far away from the dash (see UPDATE below). But while we don't know which seat positions could cause the problem, we do know that the result could be serious. NHTSA says that, in the event of a crash, the seat belt may not adequately protect the front seat occupant. Th agency's release states that there are up to 3,176 units affected by the safety action. Panamera owners (congratulations!) can take their vehicle to the nearest Porsche dealer, where an additional locking element will be installed onto the belt anchoring system to correct the issue. To assure the front belts works properly in all possible positions, Porsche will add an additional element to the anchoring system. The repair takes about 15 minutes and will be done free of charge. No injuries or fatalities have been associated with this recall."
  18. Apr 7, 2010 Nissan recalls 25,000 cars TOKYO - CARMAKER Nissan is to recall more than 25,000 vehicles in Japan due to accelerator pedal defects, the company said on Wednesday. The recall of 25,024 vehicles came on top of the company's worldwide recalls for nearly 540,000 vehicles, most of them in the United States, over brake pedal defects and faulty fuel gauges. The company decided there was a risk that gas pedals in affected cars could return slowly from a depressed position due to oil clogging up the controlling mechanism, Nissan spokesman Mitsuru Yonekawa said. 'All the vehicles subjected to the recall are equipped with the brake override system, which stops the car even when the gas pedal hasn't completely returned,' he added. The latest recall covers five passenger car models - the Cefiro, Bluebird, Sunny, Primera and Tino - manufactured between Aug 1998 and Aug 2002 and only affects the Japanese market, Mr Yonekawa said.   19. In the latest blow to a Japanese automaker, Honda will recall 412,000 vehicles to fix a faulty braking system. Consisting of 344,000 Odysseys and 68,000 Elements, from 2007 and 2008, the recall aims to fix "soft" brake pedals. When the 'vehicle stability assist modulator' tests itself on startup, a small amount of air is allowed into the hydraulic brake lines. Over time, the air can build up and cause the brake pedal to require more force to get the vehicle to stop. According to Honda spokesman Chris Martin, "It's definitely not operating the way it should, and it's safety systems, so it brings it to the recall status." Owners will receive a letter in the mail, by the end of April, with additional information about the recall. If your vehicle already has a "soft" pedal, Honda dealers can bleed the braking system which should temporary fix the problem until recall parts (plastic caps and sealant) are available.   21. Honda announced the voluntary recall of over a half a million Fit compact cars for a faulty part that has the potential of catching fire. The company said that 646,000 of their 2007-2008 Fit compact hatchbacks would be called back to dealerships so the company can examine window switches. If exposed to liquid, the faulty switches could smoke, melt or catch fire. "There have been 7 confirmed cases in the U.S.," said Honda spokesman Chuck Schifsky. "In two cases those have involved fires." Schifksy confirmed that in the seven reported incidents, no injuries or deaths were reported. Mostly commonly the problem occurs when a driver leaves a window open, allowing water to drip into the window switch module. "Under some severe operating conditions, water, rain, or other liquid may enter the driver's window and reach the master power window switch, resulting in impaired function of the switch," the Japanese car maker said in a statement. "If the master power window switch is damaged as a result of the liquid intrusion, it may result in failure of the switch and overheating. "An overheating switch may cause smoke, melting or, potentially, fire," it added. Honda says that only about 10% of the total number recalled will need new switches. The others will receive a new waterproof gasket to prevent leakage into the switch area. Of the total number of vehicles recalled, about 140,000 of them are located in the U.S. Outside of the U.S., the Fit is sold under the name Jazz.

In a statement, Proton said there was a possibility of water entry into the rear wheel bearing that could lead to a malfunction of affected components. 'As a precautionary measure, we are recalling all Savvys for inspection and repair,' Proton Managing Director Syed Zainal Abidin Syed Mohamed Tahir said in the statement issued through the national news agency Bernama. The statement did not give details of any complaints reported so far. The company urged Savvy owners to bring their vehicles for free inspections at its centers. -- AP

TOKYO: Toyota Motor Corp. said Wednesday it would recall more than 630,000 minivans in Japan due to defective fuel tanks and computer systems, in the latest quality concern to hit the auto giant. The latest recalls will cover 628,239 Noah and Voxy minivans produced from November 2001 through May 2007 because of defective fuel tanks that may crack, the company said. The company also issued a recall for 2,617 Noah and Voxy minivans with cruise-control systems that may stall when the accelerator is pressed rapidly. Toyota, on course to overtake US giant General Motors as the world's top selling automaker, has announced several large recalls in Japan and abroad in recent years, prompting a renewed push by executives to resolve quality issues. Japan's fifth-largest automaker is recalling a total of 170,300 Demio, Verisa and Accela passenger vehicles in Japan produced between March 2005 and October 2007 due to errors in the system controlling the engine. The company said the glitch could cause drivers to experience difficulty trying to slow down when releasing the accelerator pedal. The company said it had received 16 complaints in Japan although no accidents have been reported. Mazda also expects to recall 17,000 of the vehicles in Australia and Singapore, the main export markets for the brands, a company spokesman said. It is also recalling in Japan 136 Titan commercial trucks produced between June 1995 and February 2000 due to a potential problem linked to the brake booster.
