Chargers that are designed to charge Lithium Ion batteries usually fast charge the battery to about 80% and then trickle charge from then on to full. Trickle charging is supposed to minimise thermal dissipation which is bad for the service lifespan of the battery i.e. number of lifetime charging cycles and eventually battery stamina. It seems to me that your phone is draining the battery capacity at a higher rate than the latter can be charged in the trickle charge phase. A probable cause is that your phone is being kept awake. We call this a wakelock. A wakelock is a software method for applications to control the power state of your device. The purpose of a wakelock is in its name, to lock your device in an awake state keeping the CPU running and preventing it from entering Deep Sleep (Think of a music app or video player). A wakelock can also cause your device to turn on (think syncs or notifications). Two main kinds of wakelock. 1.) Screen is lit and CPU is running. Kernel Wakelock 2.) The solution for this is: shutdown your Note, peel off the back cover, remove the battery; hold the battery for more than one min, put the battery back, put back the back cover; start the phone.
